| eClinicMD New Patient Registration Form |
If you are a new patient to our service, please proceed through the registration
process. There is no charge or fees associated with registration. You only pay
for your consultations with the doctor. Once registration is complete, you will be
asked to schedule a face to face appointment with one of our physicians (After your
face to face appointment, your follow-up appointments will be performed remotely
through telemedicine so that you do not have to make continual trips to the doctor's
office). After you schedule a consultation, you will be asked a series of questions
about your medical history. Every step of the way, you will receive emails
confirming and notifying you of the registration and consultation process.
